Monogram Order Form

PLEASE NOTE: Due to the impending birth of triplets, I will be on maternity leave from approximately Dec 1, 2007- March 1, 2008. I will not be taking orders during this time, but if you are inquiring about future work, please just send me an email and I will answer it as soon as possible.

Answer these easy questions so I can get an idea of the style you'd like for your monogram

Once you have submitted this form, I will send you a PayPal invoice for $20 for one monogram. If you choose to purchase more than one design, I will send you an additional invoice at the end of the design process. Upon payment, I will start working on your monogram. I will present you with 4-5 different designs. You can then select the monogram(s) you would like, and they will be sent to you in whatever format you need - JPG, TIF, EPS, PDF, etc.

Price: $20 for the first monogram, $7 for each additional monogram (from presented designs.)

If you don't like the designs you're presented with, let me know what you'd like and I'll present you with more designs.
